Student Account Form
Each student who registers needs one Student Account Form. Please note that this form does not mean your student is registered. The $15.00 registration fee must be paid in full with the Student Account Form to be considered enrolled. No student may register on the day of the first class.
Tuition is due in full by April 1, 2018. Each month, there is a $20.00 minimum fee that must be paid on your tuition bill by the first class of each month. We understand that financial situations do happen. Please notify BBB4E if a payment will be late. If no notice is given, a $10.00 late fee will be charged to your account.
Family Agreement Form
Each family will need to turn in one Family Agreement Form per family, not per student. This form may NOT be faxed. The original copy with the parent/guardian signature is required to be considered enrolled.
No student may participate without the Family Agreement Form turned in.
Each year, our families are given a paper calendar.
Additional performance opportunities come to BBB4E throughout the school year and may be added to the calendar. We ask that all our families stay in constant contact with staff, especially if your student misses a class. Permission slips and information go home regularly with your student and are emailed the following day.
Other educational opportunities may arise for your student for visual art, literature, writing, etc. BBB4E music students are always given the first chance to sign up for any BBB4E class before it is announced to the public. For more information, contact us at email@example.com
Hammond Voucher Application
The Hammond Voucher Application is for Hammond residents only. Hammond will award any resident who applies and meets the qualifications 50% of tuition paid for.
No one may apply to the city without being fully registered with BBB4E. BBB4E will give families a receipt form that proves students are registered. This is the only way for the city to approve your application.
DEADLINE: No one may apply to the Hammond Voucher after November 1, 2017.
Non-Hammond residents may apply for tuition assistance through BBB4E. The application is extremely similar to the Hammond Voucher and follows its guidelines and income numbers.
